Registration

Student Enrollment Requirements for the Amity Regional School District are the following:

• Parent/Guardian Photo Identification (If the mortgage is in one parent’s name, please use that ID)
   
• Student's Birth Certificate and/or Passport (only if birth certificate isn't available) Showing Full Name and Date of Birth
   
• Unofficial Transcript (Most Recent Report Cards and Standardized testing)
   
• Health Records - Including Immunizations and Physicals.
   
• Two Proofs of Residency From the List Below:
   
  • Proof 1: Monthly Mortgage Statement, Deed, Lease Agreement, Purchase Agreement or Building Contract.
    
    Monthly Mortgage statement must be within the current 90 days. It must show the homeowners name, date and if possible the wording Property Address: address listed. Lease Agreement cannot be expired, must be updated yearly.
     
  • Proof 2: Renters' Insurance, Homeowners Insurance, Voter Registration Card/Letter,  Real Estate Tax Bill, Utility Bills, Utility Company Confirmation of Service Letter (Example: Wifi, Electric, etc.)
    
    All statements must be in the homeowner’s name, dated with in the current 90 days, Insurance statements must show property address that is insured and must show the insured dates.
     

Additional Required Documents If Applicable:

• Legal Name Change Document
• Guardianship Papers
• Policy #5118 Affidavit Forms 5118.1 or 5118.2 (If student does not reside with parents/legal guardian)
