To sign up for a sport, click on the high school Athletic Handbook link, print out and complete Forms #1 , #2 and #3, have the nurse stamp the bottom of Form #1 and hand in all three forms as a packet to the coach.

Students are required to obtain a Sports Permission Packet online or with the coaches on the day of the team meeting.

Physicals must be accurate and current. They will be effective for the entire season. Please see our school nurse today to check on the status of your physical examination form.
