Text Box: FALL Physical Clearance Requirements
1.        Physicals must be completed both page 1 and 2 of the State Department of Education Health Assessment Record form with parent/guardian signature, physician signature, date of examination, and physician stamp.
2.        State Department of Education Health Assessment Record form may be downloaded off Amity’s Athletic Trainer’s Wed site the link is found on Amity’s Athletics Home Page or a hard copy may be obtained in the high schools main office.
3.        Please no summer, sport camp or Boy Scout physical forms accepted. These forms will be denied by the school nurse.
4.        All physicals must be dated after November 30, 2006.
5.        Physicals can not expire during the Fall season which finishes November 30, 2007. As explained in the Student Athlete Handbook under the heading; PHYSICAL AND PRE-PARTICIPATION EXAMINATIONS
6.        All physicals must be handed into the school nurse prior to tryouts. No exceptions. The school nurse will be in the High School Health Office on the following days for physical clearances, please bring your “green” ATHLETIC DEPARTMENT MEDICAL PERMISSION TO PARTICIPATE form from the Student Athlete Handbook to be signed.
