Hello and welcome prospective Amity Teen Teachers. We are looking for dedicated Amity students who are interested in possibly pursuing a career in teaching or education. We would like to know more about you and ask that you complete the following questionnaire in paragraph style so we can get to know you better. Include a picture of you!!

•  Who are you…tell us about yourself. Your name, age, favorite food, color, musician, type of ice cream, sports interests, teams you follow or sports you play, etc…

•  What has lead you to decide to dedicate your time to working with children at the John S. Martinez School in New Haven…do you have any experience working with kids?

•  Are you aware that by joining this group you are committing to attending 8 full day trips to JSM School between October and May? Are you a dedicated type person who can make up missed work in a timely manner?

•  What age group of children would you like to work with if you had a choice? You may choose between K-6 th grade, or any of the specials like Physical Education, Art, Music or Special Education. Did you know ESL was also available? Do you speak a second language?

•  Are you a creative writer? Are you aware you will be responsible for attending 1 meeting a month and writing 1 lesson plan monthly too?
